If you would like to donate an Altar Flower arrangement (up to TWO arrangements each Sunday), please fill out this registration form.

Please select your desired date. If you are wanting to donate two arrangements, please select "2" to reserve both arrangements.

You may enter your Altar Flower message here, or you can wait and let the office know your message at a later date.

Someone from our Worship Committee will contact you the week prior to discuss your arrangement.
